At the moment you need to hurt the quest npcs below 30% to be able to use the Questitem.
Blizzlike there was a bug, looks like not fixed yet, that you can use the item at 100% life.

And a short time before they despawn they get hostile again. I think it's 0.1 or 0.2 seconds before despawn.
